Practical records. Clear answers.
Puppy Weight Log Template
A puppy weight log pairs a stable puppy identifier with a date, time, measured weight and unit. Keep entries in order and use the same unit so later readings can be compared without guessing. Add feeding or care observations in separate notes. The printable and CSV templates below organize records; they do not interpret growth, diagnose a condition or suggest a treatment.

What this record means
This is a chronological measurement record for one puppy. A litter-level summary should still preserve each puppy’s identity.
Who is this for?
Carers who want a readable history of measurements and observations.
What should be recorded?
Use this review checklist before sharing your record.
| Check | Information |
|---|---|
| □ | Puppy name and stable identifier |
| □ | Measurement date and time |
| □ | Weight and unit |
| □ | Care observation, kept separate |
Example
Illustrative record only: Puppy Blue, October 4 at 09:00, 310 g. A note identifies the scale used. This number is not a target or a statement that the puppy is healthy.
Step-by-step workflow
- Choose an identifier that remains clear across the litter.
- Write the measured number, unit and time immediately.
- Keep the original reading and date any later correction.
Common mistakes
- Mixing grams and ounces without labels makes comparison unreliable.
- A log entry cannot establish a health diagnosis.
Paper, spreadsheet or iPhone?
Paper works at a handover without a device. CSV makes rows easy to sort in Excel, Numbers or Google Sheets, but you must manage revisions and copies. An iPhone app can keep related records together; choose it when its documented workflow fits your needs. None of these formats removes the need to verify the original entry or keep a backup.

Does the sheet provide weight targets?
No. It records measurements only. Ask a veterinarian about health interpretation.
Can I use it for a whole litter?
Yes. Keep a distinct identifier on every row and filter by puppy in the CSV.
